Quick answer: Some of the strongest evidence in all of Tai Chi research is neurological. Parkinson’s disease: the NEJM trial
New England Journal of Medicine, 2012
195 people with mild to moderate Parkinson’s were randomized to Tai Chi, resistance training or stretching, twice a week for 24 weeks. The Tai Chi group did consistently better on postural stability, stride length, functional reach and timed up-and-go, and recorded 62 falls versus 133 (resistance) and 186 (stretching). Benefits were still present three months after the classes ended. The program emphasised rhythmic weight shifting and controlled movement near the limits of stability, exactly the skills Parkinson’s erodes.
Stroke recovery: 27 randomized trials
Systematic review and meta-analysis, 2022
Pooling 27 randomized trials (18 Tai Chi, 9 Qigong) in 1,919 stroke survivors, Tai Chi and Qigong produced large effects on walking ability (Hedges’ g 0.81) and dynamic balance (g 1.04) and a moderate effect on activities of daily living. The effect held even against active comparators such as physiotherapy and balance training, and no serious adverse events were reported.
Cognition in older adults: 17 randomized trials

In 2,365 older adults, Tai Chi and Qigong improved both physical and cognitive function. Crucially, the cognitive benefit remained significant after controlling for the physical improvement, which suggests the practice works on the brain directly through attention, sequence learning and breath coordination, not just by making people fitter.

How to put it into practice
- Weight shifting is the medicine. The NEJM program was built on slow, rhythmic shifts from leg to leg. Tai Chi Ruler Qigong teaches exactly this with a simple wooden aid that gives the hands a focus and steadies the movement.
- Stand near support. Practise Zhang Zhuang standing meditation beside a counter or sturdy chair. Start with one to two minutes; the trial gains came from consistency over 24 weeks, not from long sessions.
